Each phase has a different objective and helps researchers answer important questions about the safety, efficacy and use of the treatment.

But what do these phases actually mean, and why are they so important?

What are clinical trials?

Clinical trials are scientific studies designed to evaluate new medicines, medical devices, therapies or other medical approaches in humans.

They are conducted in accordance with a pre-defined protocol and are subject to ethical and regulatory oversight. The aim is to collect reliable data to help determine whether a particular treatment is safe and effective.

The various phases of clinical trials form part of this process.

Phase 1: How is the new treatment tolerated?

Phase 1 is usually the first stage in which a new medicine is tested on humans. The main question is: How does the body tolerate the treatment, and what is the appropriate dose for further testing?

Depending on the type of treatment and the protocol, trials may involve healthy volunteers or patients with a specific condition. In the case of some cancer treatments, for example, participants are patients, as the treatment may not be suitable for healthy people.

The researchers are monitoring:

How the medicine is absorbed and distributed throughout the body;

How is it metabolised and excreted;

What side effects may occur;

How the body reacts to different doses;

What dose might be appropriate for the next stages?.

Phase 1 usually involves a relatively small number of participants.

Important: Participation in Phase 1 does not mean that the treatment has already been proven to be effective. The main focus is on safety and tolerability.

Phase 2: Does the treatment work in patients?

Once sufficient information on safety and appropriate dosages has been obtained, the trial can move on to Phase 2.

The key question here is: Does the treatment have the expected effect in people with this particular condition?

Phase 2 trials usually involve patients with a specific disease or condition. The researchers assess:

Does the treatment improve symptoms or other indicators;

What is the correct dose?;

What side effects have been observed;

How does the effect compare with a placebo or another treatment, where the protocol provides for this?.

This phase often involves more participants than Phase 1.

Important: Even if the results are promising, Phase 2 does not conclusively prove that the treatment is effective. Further research is needed.

Phase 3: Is the treatment sufficiently effective and safe?

Phase 3 It is usually a larger clinical trial designed to confirm the results of the previous phases.

The main question is: How does the treatment perform in a larger number of patients and in comparison with the current standard of care?

Depending on the protocol, Phase 3 may include:

A large number of participants;

Patients from various medical centres;

Comparison with a placebo or standard treatment;

Longer-term follow-up;

A more detailed assessment of efficacy and safety.

This phase is particularly important because it enables more reliable data to be gathered on the benefits and risks of the treatment.

Upon successful completion of Phase 3, the results may be used when submitting an application for regulatory approval.

Important: Not every treatment that reaches Phase 3 is approved. The decision depends on the results and the assessment by the regulatory authorities.

Stage 4: What happens after approval?

Phase 4 takes place once the medicine or therapy has already received regulatory approval and is being used in clinical practice.

The main question is: How does the treatment perform in real-world use, and what are its long-term benefits and risks?

The following can be investigated in Phase 4:

Rare adverse reactions;

Long-term safety;

Efficacy in different patient groups;

Interactions with other medicines;

Application in various medical situations;

Data on quality of life.

This phase is important because some adverse reactions may only be identified once the treatment has been used by a larger number of people over a longer period.

Important: Phase 4 does not mean that the treatment is experimental in the same sense as in Phase 1. It forms part of the follow-up to an already approved treatment.

How do the four phases differ?

Phase

Key question

Regular participants

Phase 1

How is the treatment tolerated and what is the appropriate dose?

Healthy volunteers or patients

Phase 2

Is the expected effect observed in patients?

Patients with a specific condition

Phase 3

Have their efficacy and safety been confirmed?

A larger number of patients

Phase 4

How does the treatment perform once it has been approved?

Patients using the treatment in practice

An important clarification

Not all clinical trials progress through the phases in exactly the same way. Some may combine Phases 1 and 2, whilst others may have a specific design depending on the type of treatment.

For example, a Phase 1/2 trial can simultaneously investigate safety, the appropriate dose and preliminary data on efficacy.

What does this mean for patients?

For patients, the clinical trial phase is important because it shows what kind of information has already been gathered about the treatment in question.

For example:

At Phase 1 the main focus is on safety and tolerability;

At Phase 2 it is being investigated whether the treatment has the expected effect;

At Phase 3 the results are confirmed in a larger number of patients;

At Phase 4 Treatment is monitored following its approval.

This does not mean that one phase is „good“ and another „bad“. Each plays a different role in the development of new therapies.

Why are clinical trials important?

Developing a new medicine or treatment is a complex process. Before a treatment becomes available for widespread use in medical practice, sufficient data must be gathered on its safety, efficacy and appropriate use.

Clinical trials help to answer these questions in a structured and scientifically sound manner.

They are important not only for the development of new medicines, but also for:

Improving existing treatments;

Research into new medical devices;

Developing new treatment approaches;

Assessment of long-term safety;

Improving medical practice.

Clinical trials at the Unimed Medical Centre

В Unimed Medical Centre We take part in clinical trials across various therapeutic areas.

Our team works with pharmaceutical companies, biotechnology companies and contract research organisations (CROs), supporting the conduct of clinical trials and the monitoring of patients.

It is important to us that patients receive clear information about the aims of a particular trial, its phase and the conditions for participation.

If you are interested in taking part in a clinical trial, please contact us to find out whether we are currently conducting a suitable trial for your condition.

Conclusion

Phases 1, 2, 3 and 4 of clinical trials represent different stages in the development and monitoring of medical treatments.

Phase 1 focuses on safety and tolerability, Phase 2 on a preliminary assessment of efficacy, Phase 3 on confirming the results, and Phase 4 on monitoring treatment following approval.

Understanding these phases helps patients to better navigate the world of clinical trials and make informed decisions about their participation.
